Allergy Forms
You will find these forms useful when keeping track of allergic reactions, especially when you're keeping record for your visits to the IWK Allergy Clinic in Halifax. The majority of those forms are child oriented and are meant to help parents compile data about allergic reactions, particularly if a child has multiple food allergies and is still experimenting with new foods. The top two forms specifically have been requested by Dr. Watson at the IWK Allergy Clinic. He is doing so much for our children so let's make his job easier by providing as much information as possible to help him help us.

I compiled a series of forms into a package for parents who are in the process of writing their wills. Many of us prefer not to think about such things. The thought of leaving behind a child is dreadful in and of itself let alone one that suffers from severe multiple food allergies. The purpose of the package is to educate the person who will be looking after your child in case you are not available. When I designed it, I meant for it to be passed on to the person who will be taking custody of my child in case something happens to my husband and I. Feel free to download it and look through it.
